Access data for an online questionnaire (Ondasys portal) - simply scan the QR code with your mobile phone or tablet and fill in the questions that apply to you. This saves some time at the appointment and we can better prepare for your appointment. For optimal data protection, the data is transferred completely anonymously from the portal to us and only linked to your personal data in the practice system.

Appointment does not fit, no mail received?

If the proposed date suits you, please send us a short confirmation by e-mail. Otherwise, you can give us a time slot for the desired date.

No answer? Please understand that we are not always able to answer your enquiry at short notice if there is a very high volume of enquiries or if the practice is only staffed by one colleague during the holiday period. In urgent cases, we therefore ask you to follow up by telephone.
